Referrals are accepted via your own stationery, or you can feel free to download and complete the below form. If you would like some hard copy referral pads, please contact us.
For patients requiring acute attention and suffering from the below symptoms, we kindly ask that a referral along with the patients details be marked ‘Urgent’ and forwarded by fax or email and we will endeavor to create an emergency slot.

Sydney Hills ENT clinic is a private practice, and does not offer bulk-billing. Payment is required at the time of the treatment. The fee for an appointment is outlined to patients at the time of booking.
The clinic does not accept payment in cash. Payment following your consultation can be made using EFTPOS (we accept most major cards, including Visa and Mastercard – we do not accept American Express or cheques).
For all surgical payments by credit card, a 1.75% payment surcharge applies.
We process Medicare rebates on the spot at the time of payment on your behalf. Patients eligible for a Medicare rebate will receive their rebate into the bank account linked to their Medicare within 1-2 business days.
We are a mixed billing practice – between June 2023 and June 2024, consult fees range between $350 – $500. Fees are subject to change.
A 24-hour cancellation policy applies for appointments at Sydney Hills ENT clinic.
Please provide us with no less than 24-hours notice if you need to cancel/change your appointment. For any appointments cancelled within 24-hours, we require a flat prepayment of $450.00 if you wish to rebook with us. All prepayment fees must be paid prior to booking your appointment.
Please arrive for your appointment on time as our clinic works hard to run to time with appointments. If you are going to be late for your appointment, it is important that you contact the clinic as soon as possible. If you are more than 10 minutes late to your appointment, we will have to cancel your appointment and prepayment fees will apply.
To help manage waiting lists for consult and surgery, we use a category system to decide the urgency of patients’ health needs. Our clinic will make every attempt to treat patients within the time frame recommended by your doctor. We have limited urgent appointments available.
As our ENT specialists implement a triage system, patients who require urgent appointments or Category 1-2 surgery (30-90 day wait period) will be accommodated.
Appointment Reminders:
Once booked, we will send you reminders about your appointment via SMS. It is important that you notify us if you wish to cancel or postpone your appointment – if we do not receive confirmation of
your appointment within 24 hours of your appointment, your appointment will be cancelled.

- Medical and family medical history
- Private Health Fund Details, Medicare Card or DVA Card
